Cuisine Restaurant of the Year entry form?

The latest Cuisine mag said you could download the entry form for this year's Restaurant of the Year from their website but I can't find it anwyhere. Any message I leave on their phone or email I send aren't responded to. Does anyone know where I can get this entry form from?

Collie, I've come across this kind of problem before.

Deadlines for print are a long time ahead of issue date and an item that is referred to (like said entry form) is supposed to be on the website, but maybe whoever is responsible for administering the website hasn't put it there yet: left hand and right hand not communicating.

I see there is a review of the 2008 awards with all the results and I'm sure that the 2009 awards would have a high profile on the site if there already. It may just be a case of checking back until it eventually appears.

I must have missed seeing the bit about the entry form. Was it a pull out brochure?
Sometimes websites are lagging behind.
I did see a competition for the Artisan Award Hampers.

The entry form mention's on Page 149, March issue. Thing is, entries have to be in by 3 April.
